Job Function/Responsibilities:
1. Performs Assessments.
2. Develops assigned service plans designed to meet the personal care needs of the resident,
while promoting resident independence and dignity.
3. Assists in the resident’s medication monitoring and set-up program.
4. Assists with the development of service plans designed to meet the personal care needs of
the resident, while promoting resident independence and dignity.
5. Communicates with resident’s physician(s), other pertinent healthcare providers, and family.
6. Refers resident to appropriate medical care and conducts follow-up as needed.
7. Assigns various personal care tasks contained in the resident’s service plans to the Certified
Resident Attendant on an ongoing basis.
8. Provides health promotion services, disease prevention counseling, and teaches self-care to
resident on an assigned basis.
9. Provides general updates and progress notes to the resident’s record.

Reports to: Director of Healthcare Services
Qualifications: Minimum 1 Year experience in a geriatric care environment.
Educational Requirements: Valid Illinois RN License

Job Information

 Housekeeping Attendant

Part Time Position: 7:00am- 3:30pm Saturdays & Sundays

Reports to: Director of Environmental Services

Responsibilities:

·     Able to sweep, mop, and polish floors; vacuum carpets; clean restrooms; wash windows and walls; move and arrange furniture and equipment; pick up outside refuse.

·     Dress in appropriate uniform and be at appointed workstation promptly when the shift starts and work consistently until the shift ends.

·     Demonstrate ability to clean and sanitize areas of responsibility consistent with facility policy and procedure.

·     Assist in cleaning of emergency spills observed or upon request

·     Attend all facility and department in-service training.

·     Work in cooperation with all other employees of the facility to maintain a clean and sanitary environment for both residents and staff.

·     Maintain a safe work environment and adhere to facility safety program at all times.

·     Other duties as assigned.

Job Requirements:

Physical Requirements:

·     Able to move around the facility to carry out assignments

·     Able to lift 50- 75lbs

·     Communicate verbally and in writing with residents, families and other staff

·     Able to stand for extended periods of time

Educational Requirements

·     High School Diploma or equivalent

·     Housekeeping experience preferred

Position PRN Receptionist

Receptionist
PRN Position: Part Time – 2 weekdays 4:00pm-8:00pm & Sundays 9:00am-5:00pm
Reports to: Director of Human Resources
Responsibilities:
• At the start of each day, complete opening procedures, including: opening the reception
area, turning on all appropriate office equipment, switching the telephone off the Night
Answering Service, and listing/responding to all voice messages
• Answer and direct incoming calls
• Assist family members and visiting guests regarding information pertaining to facility
director and amenities
• Sort and distribute mail to appropriate Directors/Departments
• Some light typing, as requested, as well as bulk mail preparation(s)
• Prepare postage via postage machine for all outgoing mail
• Log all incoming deliveries made at the front desk
• Log menu orders for patio home residents and guests of the facility and forward to
Kitchen for preparation
• Create and distribute Communication sheets regarding residents, (i.e., new admission,
room change, discharge, re-admission, expiration) through email correspondence
• Update daily rosters and telephone lists, as needed
• Maintain dining reservations for Garden Dining Room and Meeting Rooms as requested
• At the end of each day, complete closing procedures, including: turning off all appropriate
office equipment, switching the telephone to the Nigh Answering Service and locking the
reception area
• Other duties as assigned

Job Requirements:
Education: High School Diploma or equivalent
Experience: Previous Receptionist or Customer Service Experience

Qualifications:

  • Ability to lift 50-pounds maximum with frequent lifting and/or carrying of objects weighing up to 25 pounds. Stooping, pushing, pulling, standing, and walking.
