Position Staff Nurse – Supportive Living Community
Job Information

Staff Nurse
Reports to: Director of Healthcare Services
Hours: Monday—Friday 8:30am—5:00pm (additional hours as needed)

Essential Functions:

  • Perform Assessments
  • Develop assigned service plans designed to meet the personal care needs of the resident, while promoting resident independence and dignity
  • Communicate with resident’s physician(s), other pertinent healthcare providers, and family
  • Refer resident to appropriate medical care and conducting follow-up care  as needed
  • Assign various personal care tasks contained in the resident’s service plans to the Certified Resident Attendants on an ongoing basis
  • Assists in the resident’s medication monitoring and set-up program
  • Provides health promotion services, disease prevention counseling, and teaching self-care to residents on an assigned basis
  • Provides general updates and progress notes to the resident’s record

Education/Experience:

  • Valid Illinois RN License
  • 1 Year experience in a geriatric care environment

Dietary Aide

Full Time: Combination of Day & Evening Shifts consisting of 6:30 am – 3:00 pm and 11:00 am – 7:30 pm; Some Weekends; & Rotating Holidays

Location: Tabor Hills Supportive Living Community | 1439 McDowell | Naperville, IL 60563

Overview of Position:

Performs a variety of tasks in the preparation, service, and clean-up for meals served to residents, families and facility staff.

Job Function/Responsibilities:

  • Works on meal tray serving line. Checks meal trays for accuracy before they leave the kitchen area. Assists Cook, as needed.
  • Washes pots, dishes and silverware according to dishwashing, pot washing procedure. Puts clean pots/dishes and silverware away. Cleaning of equipment and kitchen floor/refrigerators as assigned.
  • Maintains clean, neat, and orderly work areas.
  • Takes trash out to dumpster.
  • Carries pans, kettles, and trays of food to and from work stations, stove, and refrigerator.
  • Pushes food carts to and from appropriate nursing wings.
  • Prepares nourishments according to facility procedures.
  • Utilizes supplies and equipment properly and without waste.
  • Thinks and acts calmly and logically to meet unusual occurrences of the job without being thrown off stride.
  • Performs any miscellaneous work assignments, as may be required.
  • Notifies Director of Culinary Services or Supervisor regarding supplies of poor quality or inappropriate items.
  • Ability to lift 50 pounds maximum with frequent lifting and/or carrying of objects weighing up to 25 pounds. Stooping, pushing. Pulling, standing and walking.

Reports to: Director of Culinary Services

Educational Requirements: Ability to read and understand directions.

Admissions Associate (Part Time) – Healthcare Facility

Reports to: Director of Admissions

Hours: Saturday 9am — 4pm | Sunday 9am — 4pm (Please feel free to apply for any or all of the available days)

  • Provide prospective residents and/or responsible parties with appropriate information and assistance for admission to the nursing facility.
  • Conduct tours and process all inquiries, i.e. gathering all necessary materials for admission determination & coordinates information with Medicare team for eligibility of Medicare admission.
  • Compile all necessary information for possible admissions through interview process.
  • Organizational skills necessary.
  • Sales/Marketing experience, particularly in healthcare services preferred. Familiarity with long-term care and/or healthcare services helpful.
