The Staff Nurse will be responsible for, within the scope of practice, coordinating the delivery of care for residents in accordance with professional standards of practice, physician’s orders, facility policies and procedures, as well as local, state and federal regulation.  Primary duties include:

·     Administer direct resident care; provide treatments, administer medications, provide resident education and training, and oversee daily care routines.

·     Assess, or participate in the assessment of, resident’s care needs, strengths and potential

·     Plan, or participate in the planning of, interventions to maximize resident’s abilities to optimal level of function through the use of comprehensive assessment data base.

·     Monitor compliance with quality care standards, restorative protocols and local, state, Federal guidelines through documentation and auditing.

·     Make frequent rounds to evaluate effectiveness of care delivery to ensure compliance with quality care standards.

·     Assist staff in employing methods to meet the resident’s spiritual and psychological/social needs in conjunction with social services.

·     Monitor resident nutrition and hydration, and assist residents who cannot eat independently.

·     Ensure effective infection control practices and identify potential environmental hazards

Qualifications:

·     Current/valid licensure as a Registered Nurse or a Licensed Practical Nurse in the State of Illinois.

·     Prior experience working as a licensed nurse in a long-term care setting preferred.

·     Candidate will possess the ability to communicate effectively with residents and staff. Ability to read and follow written instructions and document care given to residents in the EHR (Electronic Health Record).

Job Description:

The Infection Control Preventionist is responsible for management and oversight of the facility infection prevention and control program, including surveillance for specific health acquired infectious (HAI) diseases; reporting of cases involving the possible spread of infection; outbreak investigation analysis and interpretation of data concerning epidemiologic investigations; institution of control measures; and program evaluation.  Specific duties include:

·     Collects, records and analyzes data on healthcare acquired or associated infections (HAI) based on facility needs, devise target areas for HAI surveillance and make recommendations for control measures or interventions.

·     Assists with infection control services for resident care areas by applying epidemiologic principles to decrease resident risk for infection in a cost efficient manner.

·     Educates individuals/small groups regarding adherence to principles of Body Substance Precautions and other specific infection control precautions as needed for outbreaks.

·     Conducts in-service education classes on infection control and prevention, antiseptics and disinfectants, specific infectious disease and other related health care issues.

·     Initiates outbreak investigations and reports to appropriate government agency when necessary.

·     Reviews laboratory reports and communicates possible communicable disease.

·     Assists in development and administration of follow-up monitoring procedures for residents who are exposed to communicable diseases.

·     Assists with the establishment of standards of infection control practice that is scientifically based and consistent with regulatory requirements.

·     Assists with the review and recommendation of medical products and engineering control required for infection control safety standards.

·     Routinely observes staff perform proper technique for the control of infection.

Qualifications:

·     Bachelor’s Degree in nursing, microbiology, or public health (with concentration in epidemiology), or a related field such as Medical Technology. Master of Public Health desirable.

·     Three (3) or more years of experience in infection prevention and control, clinical nursing, clinical microbiology, epidemiology or equivalent.

·     Certification in Infection Control (CIC) preferred.

·     Membership in Association for Professionals in Infection Control and Epidemiology or the ability to obtain within three (3) months of hire.

Job Description:

The Cook reviews menus and work orders to determine type and quantities of food to be prepared. Prepares meals according to planned menus and standardized recipes in a sanitary manner to ensure the utmost quality with a minimum of waste. Reviews menus to identify all foods required for therapeutic diets before preparing foods using nutritionally equivalent substitutes when required.

The Cook operates a variety of kitchen utensils and equipment, such as blenders, mixers, grinders, slicers, and tenderizers. Bakes, roasts, broils, and steams food items to prepare for consumption. Washes, peels, cuts, and shreds vegetables and fruits in preparation for use. Tastes and observes food by tasting and smelling for palatability. Meets meal schedules. Serves meals according to portion control. Slices portions of meat individual servings, adding gravies, sauces, and garnishes as required.

The Cook maintains the Dining Services Department according to established rules and regulations. Directs and assists in performing daily and scheduled cleaning duties. Assigns duties to dining services aides as needed. Stores food in designated areas, utilizing knowledge of temperature requirements, food spoilage, and food sanitation. Utilizes supplies and equipment properly and without waste. Thinks and acts calmly and logically to meet unusual occurrences of the job without being thrown off stride. Performs any miscellaneous work assignments, as may be required. Notifies Director of Dining Services or Supervisor regarding supplies of poor quality or inappropriate items.

Qualifications:

·     Ability to lift 50-pounds maximum with frequent lifting and/or carrying of objects weighing up to 50- pounds. Frequent stooping, pushing, pulling, standing, and walking required.

·     Ability to follow time schedules for meal preparation and dining service.

·     Maintain a neat, clean, well-groomed appearance.

Overview of Position: Prepares and serves the evening meal and other foods as required, to support the menu and other facility events.

Job Function/Responsibilities:

1.     Works at meal serving station in resident dining room.

a.

Transports prepared food from kitchen to serving station.

b.

Prepares meal trays and checks for accuracy before they are served.

c.

Clears tables and prepares dirty dishes for transport back to the kitchen.

d.

Cleans serving area after meal service.

2.     Washes pots, dishes and silverware according to dishwashing, pot washing procedure.  Puts clean pots/dishes and silverware away.  Cleaning of equipment and kitchen floor/refrigerators as assigned.

3.     Prepares salads/desserts, preps vegetables for the next day and assists Cook as needed.

4.     Maintains clean, neat, and orderly work areas.

5.     Takes trash out to dumpster.

6.     Carries pans, kettles, and trays of food to and from work stations, stove, and refrigerator.

7.     Transports food carts to and from appropriate nursing wings.

8.     Prepares nourishments/snacks according to facility procedures.

9.     Utilizes supplies and equipment properly and without waste.

10.     Thinks and acts calmly and logically to meet unusual occurrences of the job without being thrown off stride.

11.     Notifies Director of Dining Services or Supervisor regarding supplies of poor quality or inappropriate items.

12.     Performs other duties/work assignments as may be required and assigned.
